Download the forms, complete the forms and save them to your computer. Then email the appropriate International Officer and attach your completed form.
- Expense Report Use this form to submit to IEO your expenses incurred while completing Fraternity District business.
- District Leadership Meeting Report Complete this form when you have completed the District Leadership Meeting (year one of the triennium), or have personally contacted each chapter president in your district.
- District Conference Report Complete this form when your District Conference is completed. Be sure to attach supporting documentation when mailing or emailing the report to the appropriate International officer. (year two of the triennium)
- Alumni Chapter Review Form Complete this form when you have completed an alumni chapter visit for each chapter in your district. (year three of the triennium)
